What if I haven’t accessed my Provider Online Account in the last 180 days?
If you have not accessed your Provider Online Account in the past 180 days, you will need to create a new account.

registration page and provide the following information for as many users as
you need.
- Facility/practice name
- Tax ID number
- Individual user’s name, contact details, and level of access
